University Project- Please Help!

Hi everyone,
My name is Jamie and I’m a university student in Australia. For one of my assignments this semester I am required to design and undertake a health research project. For my project I have decided to investigate the relationship between religious beliefs and health perception, or peoples ideas and understandings of health. The goal of this research is to raise awareness of the influence spirituality has on ideas of health so health workers can take religious patients' views of suffering and healing into account when providing treatment and not take them for granted which is often the case.

In order to complete my study I am required to get participants of a variety of religious denominations to fill out a brief questionnaire which I have written. I need 20 participants from each religious denomination and would much appreciate if some of you could complete the questionnaire below for me. I need participants who attend religious services, so if you attend a Buddhist temple or meditation sessions etc you are just who I’m looking for! If you could please repost the questionnaire below and delete all the responses that don’t apply, i.e. leave the one response which is your answer for each question, it would be much appreciated. It should only take a few minutes.

    Hello Jamie, and welcome!
    I replied to your questionnaire on another forum, so there's little point me doing so here. But You'll obtain a lot of very good replies here too.

    Folks:
    Jamie is aware of the absence of a deity, in Buddhism, and as such has re-phrased some of his questions, particularly the one regarding prayer; he is also, I believe, aware of the stance of Buddhists, on 'belief' - taking faith as confidence, rather than as a hope of a Divine source....

    Let's make the fella feel right at home!
